Management and Program Analyst

Serves as Management & Program Analyst, providing support to Primary Care Service Line operations at a Health Care System (HCS) within VISN 23 (V23). The incumbent of this position is located under the Administrative Officer/Health System Specialist (Primary Care) and has primary responsibility as the Patient Centered Management Module (PCMM) Facility Coordinator for all Primary Care services with the V23 HCS and associated Community Based Outpatient Clinics (CBOCs). The incumbent provides high level operational oversight and management expertise of the PCMM Program and has responsibility to engage in developing, analyzing, evaluating, and advising on and/or improving the effectiveness of methods and procedures, organization, staff utilization management controls, information and documentation systems or similar functions of management. The incumbent has responsibility to engage in panel management and efficiency of work, research and data analysis work, and training programs related to Primary Care and Patient Care Service lines. Duties include, but are not limited to:
Serves as PCMM Facility Coordinator (PFC) for Primary Care Teams within a V23 HCS and its associated CBOCs. This includes the VA staffed and contracted CBOCs. The incumbent has primary responsibility for management of PCMM software program; performs management, and workload and program analyses. Work includes the interpretation and analysis of a considerable amount of policy and guidelines which involve a wide variety of administrative, program management and coordinating issues in support of primary care systems. The incumbent is responsible for implementing the PACT Model and capacity formula, understanding how primary care systems and software work, and continuously monitoring, changing, and validating panel sizes independently. The incumbent is responsible for making decisions independently regarding appropriate methods to be followed to address Medical Center needs and concerns. The PFC has management and oversite of the PCMM Web Program, which involves a complex, nationally implemented, stand-alone computer program that is designed to monitor and manage primary care services throughout the VA system. The incumbent is independently responsible for ensuring the integrity and accuracy of the PCMM database, which is used to organize and manage the HCS primary care network and is reported directly to the VISN monthly. The incumbent is required to work with clinic support staff, clinic managers, clinical service chiefs, CBOC staff, MCAA staff, VISN staff and headquarters staff to maintain the integrity of this program and data. The incumbent is responsible for analyzing and interpreting applicable PCMM and HBPCMM data to compile regular primary care panel reports which include information regarding the capacity of all primary care providers' panels. The incumbent will use this information to make recommendations to Primary Care and Home Based Primary Care leadership regarding clinic access issues. The incumbent assures that Change of Provider forms are completed timely. In addition, the incumbent supervises activities performed by employees who have access to PCMM Web and performs in depth quality assurance to all actions performed by these individuals. The incumbent provides high-level operational support in the implementation and ongoing management of HCS and CBOCs PACT Programs. The incumbent will use performance data to identify areas for process improvement projects and works with Primary Care and clinic site leadership to identify any necessary resources for the successful implementation and management of such programs. The incumbent maintains communication with clinic site leadership at all CBOCs and sites of care to ensure timely responses and resolution to any PACT and/or PCMM issues. The incumbent develops, designs, and completes reports related to PCMM Web and Primary Care. This responsibility is complicated by continually changing programmatic goals and strategic priorities. The incumbent utilizes the data to analyze and evaluate the performance, effectiveness, and efficiency of the PCMM Web program. If data sources or reports do not exist, the incumbent collaborates with Ol&T, Clinical Applications, MCAA and/or other services to develop databases tailored to PCMM Web and Primary Care.
